What is RTM?

Remote Therapeutic Monitoring (RTM) is a Medicare-covered service that allows providers to monitor non-physiological data related to a patient's musculoskeletal, respiratory, cognitive, or behavioral health conditions using approved digital devices or mobile apps.

Unlike RPM (Remote Patient Monitoring), which tracks vitals like BP or glucose, RTM supports therapy adherence, symptom tracking, and functional status – even without biometric sensors.

Who Is Eligible?

CCM is designed for Medicare and Medicare Advantage patients diagnosed with 2 or more chronic conditions expected to last at least 12 months, such as:

Hypertension, Diabetes, CHF

COPD, Asthma, CKD

Depression, Anxiety, Dementia

Arthritis, Obesity, Chronic Pain
